You will need to use MassDEP’s NOI Wetland Fee Transmittal Form to calculate the Town’s portion of the state filing fee, if applicable. This fee is in addition to the Town fee listed above.
The Conservation Commission requires a total of ten copies of all plans and application documents (one original and nine copies) for Requests for Determination of Applicability, Notices of Intent, Amended Order of Condition Requests, and Abbreviated Notices of Resource Area Delineation. Please refer to appropriate checklists for specifics.
• The Commission requires an electronic submittal of the application documents and plans through the Town’s online permitting program in addition to the hard copies.
• Payment can be made through the online application (preferred). If a check is submitted, please make payable to the “Town of Dennis”.
• Please be sure to include a copy of the notice to abutters and proof of mailing.
• Permission to Access Site Form needs to be signed and included in the application packet.
• R.D.A. or N.O.I. Checklists should be reviewed prior to submitting all required information.
